What is your Skin’s Scaffolding and why does it affect aging so much?

Johanna Stirling talks about this key component of aging and how age-related degradation can be tackled.

When we think about skin ageing, we tend to focus on what we can see: lines around the mouth, jowls beginning to form, cheeks looking less firm, or skin that simply doesn’t seem to sit where it used to.

But many of these changes don’t begin at the surface of the skin.

They begin underneath it.

At Stirling Skin, we often describe the deeper supporting structures of the face as its scaffolding. Just as any structure relies on an internal framework to hold everything in the correct position, our skin relies on a complex network of retaining ligaments, collagen, elastin and connective tissue to provide support, strength and flexibility.

Understanding what happens to this scaffolding as we age helps explain something very important:

Ageing skin isn’t simply skin that has developed wrinkles. It is skin whose underlying support system has gradually changed.

Think of your face like a beautifully constructed tent

Imagine a well-made tent.

For the tent to stand tall, smooth and properly supported, several different components need to work together.

First, there are the tent poles. These provide the main structural support and help hold everything in position.

Then there are the guy ropes. They provide additional structural support and tension, helping to keep the framework stable and the tent properly positioned.

Finally, there is the tent fabric itself. Imagine that this material contains flexible, elastic fibres that allow it to move and stretch while still springing back and remaining smooth and taut.

Our facial tissues have their own sophisticated version of this support system.

We can think of the retaining ligaments as the tent poles, providing important anchor points and structural support.

Collagen is like the network of guy ropes, providing strength and tension throughout the tissue and helping maintain the integrity of the structure.

And elastin is like the elastic component woven into the tent fabric, helping the skin remain flexible while allowing it to return towards its original position after movement.

When all three are working effectively, the skin is supported, firm and resilient.

As we age, however, every part of this system begins to change.

Ligaments: the tent poles supporting the face

Deep within the face is a network of retaining ligaments.

These fibrous structures act as important anchor points between different layers of facial tissue and help hold the soft tissues of the face in position whilst gravity is trying to pull it all down. 

Think of them as the tent poles within our facial scaffolding.

When we’re younger, this support system helps maintain the architecture of the face and keeps the overlying tissues well supported.

As we age, however, the facial support system changes. Ligaments can become more lax or elongated, while the tissues around them are simultaneously changing.

Imagine the poles of our tent gradually becoming less supportive.

The tent hasn’t suddenly collapsed, but the fabric is no longer being held in exactly the same position.

And gravity continues acting on that structure every single day.

Over time, the tissues can begin to descend and gather differently across the face.

Collagen: the guy ropes providing strength and tension

Collagen is one of the major structural proteins within our skin and connective tissues.

If our retaining ligaments are the tent poles, collagen is the network of guy ropes helping to support and stabilise the structure.

Those ropes don’t simply hold the tent up. They create tension, distribute forces through the structure and help keep everything firm and properly positioned.

Collagen performs an equally important structural role within our tissues.

Young skin contains a dense, organised network of collagen fibres, providing strength, firmness and support.

As we age, collagen production declines and existing collagen becomes increasingly fragmented and disorganised. The density and quality of this supportive network therefore reduces.

Think about what would happen if our tent gradually had fewer guy ropes — and some of those remaining ropes became less effective.

The poles may still be there and the fabric may still be intact, but the whole structure would become less taut and less well supported.

This is similar to what happens within ageing skin.

Elastin: keeping the fabric flexible and taut

The third important part of our scaffolding is elastin.

If collagen provides strength, elastin provides stretch and recoil.

Returning to our tent analogy, imagine that elastic fibres are woven directly into the tent fabric.

They allow the material to flex and move without permanently stretching out of shape. When the fabric is pulled or moved, those elastic fibres help it return towards its original position, keeping the surface smooth and taut.

Elastin performs a similar function within our skin.

Every day our faces move thousands of times. We smile, talk, laugh, frown and make countless tiny expressions.

Healthy elastic fibres allow the skin to accommodate that movement and then recover.

With ageing — particularly when combined with factors such as cumulative UV exposure — the elastic fibre network changes and becomes less effective.

So the skin doesn’t simply become less firm.

It also becomes less efficient at springing back into position.

What happens when the whole scaffolding begins to weaken?

Now imagine our tent again.

The poles aren’t providing quite the same support.

There are fewer strong guy ropes maintaining tension.

And the elasticity within the fabric isn’t quite what it used to be.

No single component has disappeared. But the relationship between all of them has changed.

The result?

The fabric becomes looser.

It starts to drop.

It can gather in places.

And eventually, it begins to fold.

This gives us a simple way of understanding many of the changes we associate with facial ageing.

Our skin hasn’t simply developed a line or wrinkle in isolation. The structure supporting it has changed.

Why do folds, jowls and marionette lines appear?

Once we understand the scaffolding, visible ageing becomes much easier to understand.

When the deeper structures become less supportive and the skin itself becomes less firm and elastic, the overlying tissues can begin to descend, gather and fold differently.

This can contribute to:

  • Nasolabial folds becoming more pronounced between the nose and mouth
  • Marionette lines developing from the corners of the mouth towards the chin
  • Jowling developing along the jawline as skin gathers
  • Loss of definition through the lower face
  • Looser skin beneath the chin and around the neck
  • Fine lines becoming deeper creases and folds
  • A general feeling that the face has begun to drop or sag

Importantly, these visible changes don’t happen because of one single ageing process.

Changes in the skin and its supporting structures occur alongside changes in facial fat compartments, muscle and even the underlying bone.

This is why simply treating an individual line doesn’t always address the underlying reason that line has appeared.

The line may actually be the visible symptom of a much wider structural change underneath it.

Why skincare alone can’t rebuild every layer

We are passionate about good skincare at Stirling Skin.

Protecting the skin from UV exposure, using appropriate active ingredients and maintaining a healthy skin barrier can make an enormous difference to skin quality and the way our skin ages.

But topical skincare primarily works within the more superficial layers of the skin.

It cannot physically reach every deeper structure involved in facial ageing.

For clients concerned about laxity and loss of support (deeper folds, gathering skin and Jowling), we therefore sometimes need to think beyond the surface and ask:

Can we encourage the deeper supporting tissues to remodel themselves?

This is where energy-based treatments such as HIFU — High-Intensity Focused Ultrasound — can become particularly interesting.

HIFU: stimulating the scaffolding from within

So, to understand what the HiFu does, let’s think about our tent hanging upside-down. The key supporting ligaments (the tent poles) become stretched downwards with gravity. And, as described above, your skin (the tent fabric) gathers, and the bottom of the upside-down tent and folds develop in its fabric.

The HiFu works in two stages. First is the instant-lift on the day of treatment. The HiFu shortens the poles (ligaments) brining the whole structure upwards, and with collagen and elastin being attached to the poles, your skin is also lifted.

The second stage is the skin-tightening and happens over the next few weeks and months following treatment. The energy from the HiFu causes your skin cells to massively step up the production of collagen and elastin, and over time, this really tightens your skin around the newly lifted structure. New skin cells are also brought to the scene and these also begin producing collagen and elastin. And, amazingly, these new cells begin replicating, so you have even more cells pumping out collagen and elastin. All this collagen and elastin is melding around the ligament scaffolding – think of new, shorter guy ropes being put up all the time and more and more elastic being spread throughout your skin (the tent fabric!). 

The end result is lifted skin, tightened skin and more supple and firm skin. Gathered skin that caused Jowling and folds is pulled upwards so your features, such as your jawline and cheeks, become really nicely defined. 

All of this is completely natural – it’s exactly what youthful skin does, the energy from the HiFu is just triggering your skin to behave as it did when you are younger. 

Getting the full results from a HiFu treatments is a gradual improvement in the firmness and support of the tissue, so that you still look like you — simply firmer, more supported and refreshed.

Not every ageing face needs the same treatment

This is perhaps the most important part of understanding facial ageing.

A nasolabial fold isn’t always “a filler problem”.

A jowl isn’t simply “loose skin”.

And a wrinkle isn’t always just a wrinkle.

What we see on the surface can be influenced by skin quality, collagen loss, changes in ligamentous support, facial fat, repetitive muscle movement and changes to the underlying facial skeleton.

That’s why at Stirling Skin, we believe assessment should come before treatment.

We don’t want our clients simply choosing a machine or treatment from a menu.

We want you to understand why your face is changing, which anatomical structures are contributing to those changes, and what we can realistically do about them.

For one person, improving collagen and skin laxity may be the priority. For another, volume loss may be more important. Often, several aspects of ageing are happening simultaneously and a carefully planned combination approach produces the most natural result.

Treat the structure, not simply the symptom

Modern medical technology allows us to think about facial ageing very differently.

Rather than simply chasing individual lines and wrinkles, we can look at the face as an interconnected structure and consider what has actually changed beneath the surface.

For suitable clients, HIFU gives us a way of delivering energy into deeper tissue layers and stimulating the body’s own ligament, elastin and collagen-remodelling processes without surgery.

It won’t stop ageing — nothing can.

But by understanding and supporting the scaffolding beneath the skin, we can help suitable clients achieve firmer, better-supported and more youthful-looking skin.

And for us, understanding that science is just as important as owning the technology.
